Purchase Order Information

TraumaSFX™ is happy to accept purchase orders from the above organizations, institutions, and agencies when written and submitted on their official letterheads. They must be signed by authorized personnel and/or purchasing agents or they cannot be processed.

Please create an account in our store first, and then contact us to submit your purchase order and initiate a Purchase Order Acceptance Agreement.

If you are tax exempt, you will be required to submit your tax exempt certificate along with your Purchase Order document.

Please note that any single product volume purchase is subject to inventory availability, and may require additional time in fulfillment.

We do not accept purchase orders submitted from entities located outside of the USA.
